Membership Includes:
New Patient Consultation Fee: $200 A $50 deposit is required to start the consultation process; ordering labs, making appointment. This deposit will go towards your initial consultation fee. Consultation may take place via phone, zoom, messaging in secure portal Optimantra, or face to face with a board-certified provider. Upon completion of consultation the remaining balance of $150 will be processed. If a patient is a no call/no show for their consultation, the deposit is nonrefundable.
Existing Patient Annual Review: This will be completed approximately one year from the initial consultation. For example, if a patient starts with Top Form in June 2024, the annual review will be around on June 1, 2025. The annual review will include lab interpretation, in which orders can be sent to a lab of your choice, or we can perform the draw in our office. Lab draws done in the office are cash pay or can be charged to an HSA.
Monthly Membership Fee: $60 You will be enrolled in the LoseIt membership once a compound medication order is placed, OR weight loss medication is sent to your pharmacy. No annual contract, we do ask for a 3-month commitment, and 30-day cancelation notice.
Medicare/Senior (65+) discount: 30% off monthly and new patient fees.
First responder/Military discount: 20% off monthly and new patient fees; for first responder or military member ONLY.

Yes, we require membership in our program to continue care with us.
Yes! Labs must be done prior to consult. We are happy to send labs to your local facility (not responsible for any related out of pocket expense- please check your insurance coverage) or we can draw them in office for an additional fee of $110. Our office is located in Papillion NE.

GLP-1s include Ozempic and Wegovy (semaglutide). Mounjaro and Zepbound (tirzepatide) is additionally a GIP agonist. These are naturally occurring hormones in your gut that regulate your digestion and appetite. In short, they can help you feel full faster, and stay full longer. They are FDA approved for diabetes, and in the case of Wegovy and Zepbound, specifically for weight loss.
Well, that depends on a lot of factors. These medications can help you lose a substantial amount of weight - studies show a 10% -20% reduction in body weight over diet and exercise alone with Mounjaro and Wegovy/Ozempic. Patients taking Saxenda lose around 9%. It’s important to remember that while effective, you must also implement long term lifestyle changes with regard to healthy diet, regular exercise and healthy sleep.
If you are obese with a BMI greater than 30, or a BMI greater than 27 with at least one other condition (high blood pressure, diabetes) you may be a candidate. Those with a personal or family history of medullary thyroid cancer or MENS2 are not candidates. If you meet these criteria, you most likely would qualify!
Side effects are mild and usually go away within a few weeks of starting treatment. Most common are nausea, heartburn, diarrhea and constipation. These can usually be managed by over the counter medications, staying well hydrated, or in same cases another prescription from a healthcare provider
GLP-1 agonists and GLP-1 Agonist/GIPs are administered with a once weekly (mostly) painless injection. Saxenda is a daily injection. Oral semaglutide is a daily pill.
This will depend on a lot of factors. I'll work hard to find you cost effective solutions that will make it as affordable as possible. I can also try to work with your insurance if you have certain pre-existing conditions (such as diabetes) to try to get it covered. Saxenda, Wegovy, and Zepbound are the only GLP-1's specifically approved by the FDA for weight loss.
Well, the jury is still out. Many providers are still not sure what maintenance of your weight loss results will look like when it comes to these medications. For those that have a situational, more short term weight gain (life events like postpartum weight, quitting smoking, death in the family) it may be likely that you will be able to eventually titrate down and come off medication when you make the proper lifestyle changes. For others, for which obesity has been a chronic problem over their lifetime due to genetics, diabetes, menopause, etc, this may not be possible, and you may always require a small regular dose indefinitely. I offer nutritional and fitness support, and can refer you to excellent nutritionists, as well as support groups like Overeaters Anonymous. It will be very individualized, and I pledge to work with you to KEEP IT OFF!
